ABSTRACT

Objective:To observe the clinical efficacy and safety of decitabine combined with low-dose cytarabine in treatment of high-risk myelodysplastic syndrome.Methods:The data of 47 newly treated MDS patients who had high-risk or above scores according to revised international prognostic scoring system (IPSS-R) in the Affiliated Hospital of Qingdao University from January 2016 to September 2018 were retrospectively analyzed. The patients were divided into decitabine combined with low-dose cytarabine group (15 cases) and decitabine group (32 cases). The clinical efficacy and adverse reactions in two groups were compared.Results:After 4 courses of treatment, the bone marrow remission rate, partial remission rate and hematologic remission rate was 20.0% (3/15), 6.7% (1/15), and 13.3% (2/15), respectively in decitabine combined with low-dose cytarabine group, and was 28.1% (9/32), 3.1% (1/32), and 9.4% (3/32), respectively in decitabine group, and there were no statistically differences of both groups (both P > 0.05). The overall response rate in decitabine combined with low-dose cytarabine group was higher than that in decitabine group [93.3% (14/15) vs. 62.5% (20/32), P = 0.037], and the complete remission rate in decitabine combined with low-dose cytarabine group was higher than that in decitabine group [53.3% (8/15) vs. 21.9% (7/32), P = 0.046]. The 1-year overall survival (OS) rate of decitabine combined with low-dose cytarabine group was 86%; and the median OS time of decitabine combined with low-dose cytarabine group was 24 months (95% CI 15.5-32.5 months), which was higher than that of decitabine group (20 months), but there was no statistically significant difference ( χ2 = 0.058, P = 0.810). The incidence of grade Ⅲ-Ⅳ bone marrow suppression and infection in decitabine combined with low-dose cytarabine group was higher than that in decitabine group, but there were no statistically significant differences of both groups (both P > 0.05). Grade Ⅲ-Ⅳ bone marrow suppression and infection were commonly found within the first 2 courses of treatment in decitabine combined with low-dose cytarabine group, and the adverse reactions gradually decreased in the subsequent treatment. Conclusions:Decitabine combined with low-dose cytarabine can achieve better remission rate and prolong survival time for MDS patients with high-risk and above. There is no significant increase in the incidence of grade Ⅲ-Ⅳ bone marrow suppression and infection. For high-risk MDS patients who are not suitable or unable to receive hematopoietic stem cell transplantation, it can be the preferred option.

ABSTRACT

BACKGROUND: Exercise has been proved to accelerate the proliferation of intervertebral disc cells and extracellular matrix production in healthy rats. For the degenerative intervertebral disc, whether exercise also has positive effects on its cell proliferation, extracellular matrix production or pain relief remains unclear. OBJECTIVE: To investigate the effect of exercise on the extracellular matrix production in a rat model of intervertebral disc degeneration.METHODS: A rat model of intervertebral disc degeneration was prepared by Freund's complete adjuvant injection into the intervertebral disc at L5-6 levels. Then, the model rats were allowed to have a rest for 2 weeks. All rats were then randomly divided into exercise and control groups. Rats in the exercise group were forced to run every day, while the controls allowed free activities in the cage. The behavioral tests were performed at 7, 14, 28, 42, 56 and 70 days after modeling; meanwhile, the intervertebral disc samples were collected used for alcian blue staining and immunohistochemical staining to detect the levels of proteoglycan, aggrecan and collagen type Ⅱ in the intervertebral disc cells, respectively.RESULTS AND CONCLUSION: Vocalization threshold on the rat back of punctured disc was significantly decreased, while grooming and wet-dog shaking were significantly increased at 7 days after modeling compared with the baseline (P < 0.05), suggesting that Freund's complete adjuvant injection successfully induces disc degeneration, hyperalgesia and abnormal behaviors. Further, the vocalization threshold and wet-dog shaking in the exercise group showed significant improvement compared with the control group after 14 days of exercise (P < 0.05), while the grooming was significantly reduced until the 28th day (P < 0.01), indicating that exercise can alleviate pain caused by disc degeneration in model rats. At 21 days after modeling, the levels of proteoglycan, aggrecan and collagen type Ⅱ in the nucleus pulposus and annulus fibrosus were significantly decreased compared with the baseline (P < 0.01), indicating the occurrence of disc degeneration. After 14 days of training, the levels of proteoglycan, aggrecan, and collagen type Ⅱ in the nucleus pulposus and annulus fibrosus in the exercise group were significantly increased compared with the control group (P < 0.01). Moreover, after 8-week exercise, the level of proteoglycan in the nucleus pulposus and annulus fibrosus in the exercise group was increased by 4-5 times compared with the control group, and levels of aggrecan and collagen type Ⅱ in the nucleus pulposus in the exercise group also was increased by 3-4 times compared with the control group. To conclude, exercise can promote extracellular matrix increased by production by increasing the levels of proteoglycan, aggrecan, and collagen type II in the degenerative intervertebral disc.

BACKGROUND:Lumbar spine MRI and electrophysiological test are reliable methods for evaluating nerve root injury caused by lumbar disc herniation. OBJECTIVE:To analyze the correlation between the MRI-based grading system and the latency and frequency of F wave as wel as latency and amplitude of H-reflex in patients with lumbar disc herniation. METHODS:MRI imaging of the lumbar spine was performed with a 3.0-T imager and a dedicated TCL coil to classify lumbar disc herniation and nerve root compression. F wave and H reflex were detected on the patient bilateral tibial nerves using Oxford myoelectricity evoked potential instrument. RESULTS AND CONCLUSION:Spearman correlation analysis showed that the MRI-based grading of patients with lumbar disc herniation had a negative correlation with F wave frequency (r=-0.594 0, P<0.000 1), and a positive correlation with F wave latency (r=0.825 6, P<0.000 1) and H-reflex latency (r=0.875 0, P<0.000 1), but no correlation with H-reflex amplitude (R=0.117 4, P=0.257 3). With MRI grading increased, F wave frequency was decreased, and F wave and H-reflex latency were prolonged gradual y, indicating aggravating nerve root compression.

ABSTRACT

BACKGROUND:Microencapsulated cels are commonly used as a tool to overcome immune rejection after subarachnoid transplantation. However, the effect of microencapsulation on the secretion of human pheochromocytoma cels is unclear. OBJECTIVE:To observe the growth and secretion of primarily microencapsulated cultured human pheochromocytoma cels in artificial cerebrospinal fluid. METHODS: The human pheochromocytoma tissues were digested successively to isolate human pheochromocytoma cels that were then cultured in artificial cerebrospinal fluid. Primary cels were covered with alginate-polylysine-alginate microcapsules, and then the cel morphology was observed with inverted phase contrast microscope. Levels of met-enkephalin and norepinephrine in cel culture medium were detected by enzyme-labeled immunosorbent assay (ELISA). We used cel counting kit-8 colorimetric assay to obtain the growth curve of human pheochromocytoma cels in artificial cerebrospinal fluid. RESULTS AND CONCLUSION:Microcapsulated human pheochromocytoma cels were in suspension and the process outgrowth increased slowly. Compared with non-microcapsulated cels, the proliferation rate of microcapsulated cels increased significantly. ELISA results revealed a significant increase in the levels of met-enkephalin and norepinephrine secreted from the microencapsulated cels compared to the non-microcapsule group. There was a wide variation in contents of met-enkephalin and norepinephrine from different tumors. These findings indicate that microencapsulated human pheochromocytoma cels can survive wel and have good secretion function in artificial cerebrospinal fluid, and human pheochromocytoma cels from different tumor tissues have stable secretory function.

ABSTRACT

Objective To investigate the effect of the expression of Notch1 protein and the mutation of Notch1 gene in.T-cell lymphoma (TCL).Methods Immunohistochemistry was used to detect the expression of Notch1 protein,and PCR amplification and DNA sequencing were used to detect the mutation of Notch1 gene in the 26th and 27th HD domain and the 34th PEST domain in 30 cases.10 cases of reactive hyperplasia tissues of lymph node were as the control.Results The positive rates of Notch1 protein expression and Notch1 gene mutation were 70.0 % (21/30) and 56.7 % (17/30).8 cases of Notch1 mutations were detected in the HD domain,6 cases in the PEST domain,and 3 case in both HD and PEST domains.Inscrtion,deletion,nonsense mutation and missense mutation were included in Notch 1 mutations.Conclusion Notch1 gene mutation may play an important role in the expression of Notch1 protein.The occur of TCL is related to the expression of Notch1 protein and the mutation of Notch1 gene.

ABSTRACT

Objective To investigate awareness of knowledge about iron deficiency anemia (IDA)in patients with it.Methods Patients diagnosed as IDA at their initial visits to Affiliated Hospital of Qingdao University Medical School were randomly selected for the study during January 2008 to January 2009 with self-designed questionnaire to investigate their awareness of knowledge about IDA.Results Among 199 patients with IDA investigated, their awareness was lower in general, five percent about its etiologies, 34.2 percent about its clinical manifestations, 18.1 percent about its hazard to health, and two percent about food abundant of iron.For preventive measures of IDA, 50.7 percent (101/199) of them knew that daily diet should be diversified and 43.7 percent (87/199) knew that meals should be prepared with ferric cooking pot.Awareness of knowledge about other preventive measures of IDA was lower in them.No significant difference in awareness of IDA-related knowledge between them at varied ages was found, except for knowledge about etiology and hazard of IDA.Awareness of etiology of IDA differed between them at varied ages (P <0.05), and about 75.3 percent (64/85)of patients with IDA aged 30 -45 years were caused by chronic blood loss.There was no significant difference in awareness of IDA-related knowledge between patients with IDA caused by varied etiologies (P > 0.05).Conclusions In general, awareness of IDA-related knowledge was lower in patients with IDA.It is necessary to strengthen its popularization, with varied focuses in preventive measures for IDA patients with different etiologies.

ABSTRACT

Objective To establish the mice model of AHL, to investigate the relationship between AHL and the cytoactive factors of the cochlear hair cells in C57BL/6J mice, and to classify the presbycusis models of the C57BL/6J mice. Methods C57BL/6J mice were divided into 6 experimental groups by age (A: 3 months old(m), B: 8 m, C: 9 m, D: 10 m, F: 17 m, G: 18 m) . The auditory functions mice were measured by auditory brainstem response (ABR) with the stimulus click and toneburst at 6 kHz and 8 kHz. 3 months later, Groups C , G, E and H were tested again for ABR. After ABR testing, the cytoactive of the hair cells was detected by succinate dehydrogenase staining and surface preparation technique(two mice from each group except groups C and G). Results The ABR thresholds elevated with age, and the marked change of the cochlea was the degeneration of the cytoactive of the cochlear hair cells, especially those of the outer hair cells. In the beginning, the basement of the basal membrane suffered from the mitochondrion degeneration in the outer hair cells, then it spread to the top region. Subsequently, the inner hair cells were involved. Conclusion C57BL/6J mouse was a typical animal model for the AHL,and the main change of the cochlea was the degeneration of the hair cells, especially the outer hair cells. Thus, C57BL/6J mice can be used as a suitable animal model for the study of presbycusis.

ABSTRACT

Objective To study the effect of combination of Hydrochlorothiaside (HCTZ) with Captopril or spironolactone on serum high-sensitivity C-reactive protein (hsCRP) in hypertension patients. Methods A multi-centre, random and parallel control study was applied in this study. Slight and moderate hypertension patiens were se-lected. The patients were treated with placebo for two weeks and HCTZ 12.5 mga day for 6 weeks,wbo were then randomly divided into HCTZ group(12.5 mg once a day), spironolactone group(HCTZ 12.5 mg once a day + Spi-ronolactone 20 rag once a day) and captopril group(HCTZ 12.5 mg once a day + Captopril 25 rag twice a day) . By the end of one-year follow up, HCTZ group was randomly added to Spironolactone group and Captopril group because combination therapy was superior to single medication,which was recognized. During the treatment, the patients were followed up once a month ,for monitoring blood pressure, and serum hsCRP level was measured every year. Follow-up last for 4 years. By the end of 4 years, the patients were divided into treatment group and control group in order to compare the changes of serum hsCRP levels. Results At the end of 4 years, the blood pressure and serum hsCRP level were significantly decreased as compared with baseline, and were statistically different from that of control group (P <0.05 or 0.01). Multi-factor analysis showed that pre-treatment systolic blood pressue and serum hsCRP level, post-treatment decrease value of systolic blood pressue and age were the major influencing factors for the de-crease of serum hsCRP level(P < 0.05 for each). Conclusion The long-term combinaion of HCTZ with Spironolactone or Captopril not only effectivley decreases blood pressure but also decreases serum hsCRP level. The decrease value of systolic blood pressure is the major factor for influencing serum hsCRP level.

ABSTRACT

Objective To study the reliability, validity, sensitivity, specificity and predictive value of the Standardized Swallowing Assessment (SSA) in assessing swallowing function. Methods Sixty-seven stroke patients with dysphagia participated in the study. They were assessed twice with the SSA within 3 days. In addition, the patients' swallowing was also assessed using videofluoroscopy. Results There was high correlation in all items of the SSA between the two separate assessment sessions. The intraclass correlation coefficient was 0.85 for intrarater re- liability and 0.82 for interrater reliability. The SSA and videoflouroscopy results were highly correlated in terms of their total scores (r = -0.86,P≤0.01). Using the videoflouroscopy result as the standard, the sensitivity and spe- cificity of the SSA were 77.8% and 68.1% , respectively. At the same time, the positive and negative predictive val- ues were 48.3% and 88.9% , respectively. Conclusions The SSA has satisfactory reliability and validity, and higher sensitivity and specificity than videoflouroscopy. It is recommended for assessing swallowing with stroke pa- tients.

Pathological section showed that nerve cell and medullary sheath were integrated without fibroplasia or necrosis, and only a little of infiltration of lymphocytes appeared. CONCLUSION: Activity and secretion function can be kept by the microencapsulation technique on PC12 cells. When microencapsulated PC12 cells were implanted into subarachnoid cavity, the cells can maintain good activity and secretion without oncogenicity.

ABSTRACT

Objective To examine the validity and reliability of the abbreviated mental test (AMT) among Chinese subjects. Methods Two groups of subjects participated in the study. One group consisted of 38 patients with neurological disorders and the other 30 normal subjects (control group). They were assessed twice with AMT and mini mental state examination (MMSE) within one week. Results AMT was significantly correlated with MMSE as far as the total score and item scores were concerned( r =0.706~0.833).There were moderate to high correlation in all the items of AMT between the two assessment tools(ICC=0.810~0.996). ABSTRACT

Objective To observe the therapeutic effects of electrical stimulation of pelvic floor muscles on urinary incontinence (UI) in patients with incomplete spinal cord injury (ISCI). Methods Seven patients (5 males and 2 females; average age: 32.5 years; average duration after injury: 7.5 months) with ISCI were treated with ESPFM applied on rectum (for male) or vagina (for female) for 30~45 min,once a day, 5 days a week for (2~3) months. The rate of success on stopping using catheters and the frequency and amount of urination were observed during treatment. Results Catheters were taken out successfully from all patients 2~3 weeks after treatment. Five out of the 7 patients had more self-control of urination than incontinence after 8 weeks of treatment though there was no significant difference in the amount of the urination. Conclusion ESPFM might improve the function of urinary bladder in patients with incomplete spinal cord injury.

Objective To assess the cognitive function of subjects with brain injury with Loewenstein occupational therapy cognitive assessment (LOTCA) and mini-mental status examination (MMSE). Methods Two groups of subjects participated in this study. One group consisted of 36 subjects with brain injury (patient group). They were 28 males and 8 females,aged 61.0?16.7 years old. The other group was made of 44 healthy subjects (control group),with 25 males and 19 females,aged 55.4?23.7 years old. @#ObjectiveTo study the relationship between the activity of daily living (ADL) and the physiopsychic variables of the patients at early recovery stage after first stroke. Methods ADL and the motion activities, the deficient degree of nerve function, the cognitive function and the depressive degree of the 48 patients at early recovery stage after first stroke were evaluated. ResultsThere are significant relationship between ADL at early recovery stag and the motion activities, the deficient degree of nerve function, the cognitive function of the patients at acute period(P<0.01). Meanwhile, there are also relationship between ADL and the number of the cerebral lesion, the hemiplegic degree and the education degree of the patients (P<0.05) at early recovery stage.Conclusions The motion activities, the deficient degree of nerve function and the cognitive function of the stroke patients at acute stage are the important predictive factors to ADL of the patients at early recovery stage, and the number of the cerebral lesion, the hemiplegic degree and the educational level of the patients maybe influence ADL of the patients at the early recovery stage.
